The partnerships leverage the expertise of UK health institutions and technical experts to strengthen the capacity of the national health workforce and institutions in partner countries to address AMR challenges. The project aims to: (1) Develop partnerships between NHS Trusts (hospitals) and clinics/hospitals in LMIC Commonwealth countries; (2) Share UK expertise abroad as well as bring skills and knowledge back to NHS Trusts; (3) Create focused partnerships on the theme of AMR, that includes activity on: Antimicrobial stewardship, including surveillance (AMS); Infection prevention control; and antimicrobial pharmacy expertise and capacity; and (4) More generally improve the knowledge, skills and empowerment of healthcare professionals and pharmacists in partner institutions, thus enabling the healthcare workforce to ultimately contribute to a strengthened health system. The Fleming Fund grant specifically supports the collection and synthesis of data on the burden of disease associated with AMR in LMICs and is working to ensure that AMR is included within the Global Burden of Disease study. This project was not put out to an open, competitive tender process because IHME is the only organisation in the world with the computing power required for AMR geospatial mapping, which means they are the only institution capable of mapping the global burden of AMR into the global burden of disease. IHME is also the only institution currently mapping the global burden of disease, though the resulting information is open source access. This is a contribution to the Food and Agriculture Organization (FAO) UK Reference Centre for Antimicrobial Resistance (AMR) which is run by the Veterinary Medicines Directorate (VMD), the Animal and Plant Health Agency (APHA) and the Centre for Environment, Fisheries and Aquaculture Science (Cefas), all agencies of the UK Department of Environment, Food & Rural Affairs (DEFRA). The centre offers LMICs access to draw-down services for technical assistance, training and quality assurance to support AMR surveillance system building across all One Health* sectors. The objectives of the reference centre are to: (1) Develop LMIC capacity for surveillance of AMR and antimicrobial use (AMU) through residential AMR training workshops, country visits to partners to offer training, and provision of proficiency testing schemes; (2) Strengthen governance related to AMU and AMR by providing expert advice on new or emerging resistances that present a threat to animal or public health, providing expert advice on how to bench-mark capability and the authorisation of antibiotics and providing verification testing at our UK laboratories; (3) Promote good agricultural practices and prudent use of antimicrobials by providing advice and support for the adaptation and implementation of alternatives to antibiotics; and (4) Raise awareness of AMR by publishing and contributing to expert reports and peer-reviewed scientific manuscripts. This project is delivered in partnership with the Pan American Health Organisation (PAHO) and will strengthen efforts to combat AMR across the ten ODA-eligible Caribbean Community (CARICOM) Member States and through Barbados as a regional hub for External Quality Assurance. This will be achieved by enhancing infrastructure and building health workforce capacity in laboratories for the detection and surveillance of AMR, strengthening AMR emergency response and stewardship, and developing AMR data management and digital systems. Through catalytic investment in national systems, these interventions seek to support completion of AMR National Action Plans, increase reporting from the region to global AMR data repositories such as the Global AMR Surveillance System (GLASS) and strengthen use of AMR data to inform changes in policy, regulatory and clinical practice. This project was not put out to an open, competitive tender process because PAHO has a unique position to support in-country AMR activity and a unique mandate to support the Fleming Fund objectives on the implementation of National Action Plans.
