      "description_narrative":["The Caribbean has the highest proportion of people suffering from chronic diseases such as diabetes, cancers, stroke and heart disease in the Americas. As in other parts of the world, this is a consequence of ageing populations, urbanisation, lifestyle changes resulting from globalisation such as unhealthy diets, lack of exercise, use of alcohol and cigarettes, and other social influences. The effects of these factors on health are worse in poor communities. Health systems in poorer countries in the Caribbean face challenges in preventing chronic diseases and meeting the needs of those affected. Our approach considers how to enhance health services taking into account the wider social and cultural context in which people live. It will use the strengths and assets of communities to promote health and reach the poorest in a cost effective and sustainable way. Religion plays an important role in the Caribbean with almost everyone attending a place of worship such as a church, mandir or mosque, at least once a month. Many studies have shown that places of worship can be used successfully to promote healthy lifestyles. However these programmes are often discontinued after the research is completed. In collaboration with national and international agencies including Ministries of Health, Inter-Religious Organizations, the Healthy Caribbean Coalition, the Caribbean Public Health Agency, and the Pan American Health Organization, we will plan and implement an intervention which uses health advocates recruited from places of worship to promote health and lifestyle changes and support the management of chronic diseases. The health advocates will be linked into local primary health care centres and supervised by nurses to ensure that when needed, early referrals are made and patients are followed up in the community. By evaluating the intervention as it is implemented in real time, we will examine whether and how this approach will improve the prevention and early detection of chronic diseases as well as support the care and treatment of those already affected.  The study will take place in three of the less wealthy Caribbean Commonwealth countries which face challenges in providing quality health care: Guyana, Jamaica and Dominica. Each of these have different social and cultural features so we will be able to compare the results and develop solutions that have relevance to the entire region. We will focus on poor rural and urban areas where the need is greatest. We will work with the Ministries of Health, nurses and doctors in regional and local health care centres and hospitals, religious leaders and congregations. The Ministry of Health will train members of the congregations to be health advocates. They will be taught to conduct simple tests such as measuring blood pressure and weight, encourage people to take medications as prescribed and attend clinic appointments, inform people about welfare benefits and help with applications, and support a healthier, more active lifestyle, such as providing exercise or healthy cooking sessions. Health advocates will be supervised and monitored by nurses at local health centres who will be trained in this new role. We will conduct continual evaluation to look at if and how the intervention is working to address the needs of the community, especially the most vulnerable.  If the research is successful, we will develop a tool kit for the Caribbean providing guidelines on how health systems and communities can work together to combat chronic diseases through the engagement of places of worship and other community-based organisations. Changing the health system in this way will reach more people than through traditional health care by providing easily accessible and regular health advice and support in the heart of local communities.       "description_narrative":["In a region beset by serial disasters, effective recovery from hazardous events is not just a worthy goal it is an imperative. The Caribbean Resilience & Recovery Knowledge Network will support a transformational approach towards post-disaster recovery in the Caribbean, particularly as it responds to the impacts of the 2017 hurricane season that caused $130 billion of losses in the region. A community of researchers, practitioners & policymakers is emerging, but at the moment their response is ad hoc & driven by divergent research or short-term aid goals. This generates locally useful outcomes, that alleviate the worst symptoms from the disasters, however they are collectively less effective at encouraging long term resilience to future events, strengthening ecosystems or reducing long-term vulnerabilities in poor or remote communities. The Caribbean Knowledge Network seeks to remedy this by creating a new culture for responding to & preparing for hazardous events. This is our principle aim. We will do this by joining together diverse researchers, & engaging with practitioners & policymakers from the outset. These groups are all motivated to come together & transform how we approach post-disaster recovery. The researchers (in the Caribbean & the UK) work on wide-ranging topics relevant to long term hazard resilience, connected by their desire to improve recovery from disasters. The practitioners bear responsibility for ensuring the best possible outcomes for communities at risk & face increasing challenges associated with climate change & environmental degradation in the region. The attention of donors & policymakers has been focussed by the recent systemic destruction from the 2017 hurricanes. Thus, the Network is both timely & needed. We have structured a series of mutually beneficial activities to help us achieve our overall aim, & associated objectives. The first of these will be a forensic workshop where we will bring our multiple perspectives to bear to understand what drove & hindered recovery following three key events (the 2017 hurricanes, the Haiti earthquake & long-term recovery following Hurricane Ivan). To reflect the multiple perspectives we are calling these forensic analyses, & this will be a great vehicle for learning from each others expertise & perspectives. After that we will create some scenarios to help test, design & evaluate existing response & recovery plans for multiple types of hazard. We will visit several Caribbean countries to do this, & they will be co-designed with different social groups to ensure relevancy. These will be used to create new principles & strategies in each setting. While we are doing this we will run a series of webinars to discuss challenges highlighted by these two activities. We will make these freely accessible to participants throughout the Caribbean & further afield. The outcomes from all of this work will create our new community who will co-create a manifesto for research to transform recovery & use that to secure further funding. Importantly, however along the way we will also have created some tangible benefits which will ensure the longevity of the grouping & maximise its benefit. These are: (1) forensic analyses on several important disaster events, identifying priority actions & sets of principles; (2) flexible, tailored scenario exercises for local disaster practitioners so they can develop their own new strategies in response to these on four islands (3) the time & momentum to consider how to 'join up' strategies across the islands, critically in collaboration with regional & international agencies.       "description_narrative":["Earthquakes, storms, floods, and conflict cause untold damage to infrastructure, services, agriculture and livelihoods. The destruction of housing is often most visible, and the most devastating to the families, with often hundreds of thousands of homes destroyed. Householders themselves are invariably the first to respond; they are never passive, and signs of shelter recovery are very apparent from the first days after a disaster. The majority - as many as 80 or 90% - receive little or no assistance from the international community and are the main drivers of their own recovery process. This process has been termed 'self-recovery'. Previous GCRF awards supported interdisciplinary teams led by ODI in collaboration with CARE International UK, BGS, UCL and Loughborough University. They developed the understanding of the self-recovery process and advanced the theoretical basis for humanitarian assistance to the process of supporting self-recovery in practice. In large part as a result of this work, self-recovery is now a strategic element of the humanitarian shelter sector discourse with many disaster responses purporting to support this inevitable process. In recognition of the value of a people-centred approach that moves away from the convention product-based modalities, the CARE Philippines post-cyclone self-recovery project won the 2018 World Habitat Award in acknowledgment of its self-recovery focus. Sixteen thousand houses were built, each one unique and reflecting the priorities and means of each family. https://www.world-habitat.org/world-habitat-awards/winners-and-finalists/post-haiyan-self-recovery-housing-programme/ The self-recovery approach that respects the primacy of agency and choice aligns well with current humanitarian and development concerns such as the 'humanitarian development nexus', the Grand Bargain's emphasis on localisation, participation and cash-based responses, the Sendai Framework on Disaster Risk Reduction, the Sustainable Development Goals and the Global Shelter Cluster strategy. This is further explained in the Business Case. However, very little has been done to develop guidance for practice. With the exception of the protocol 'Informing Choice for Better Shelter' that delivers a guide for the development of technical educational material, there are no published tools nor guidelines that provide an implementation framework for self-recovery in shelter projects. This project addresses this gap. CARE International has over 70 country offices across the world, all of them in ODA eligible  countries. All of the proposed activities will take place two hazard prone countries where there is a CARE presence and/or an activated coordination system in the form of a country level shelter cluster. The potential regions to be selected include South Pacific islands, East Africa, Central America, Philippines and SE Asia the precise locations to be dependent on ongoing and new disaster responses. In all instances the research team will be working with the Global Shelter Cluster, country level clusters and/or CARE International country teams. These will all be ODA eligible countries. The protocol developed will be adapted to become a preparedness tool for the specific ODA eligible country context.
