      "description_narrative":["THE PROBLEM: Infections in wounds after bowel surgery are common in all countries. They have adverse effects for patients, doctors and health systems. For patients, they are painful, smelly, reduce quality of life and reduce time taken to return to normal activities. They are hard to treat and require prescriptions of long courses of antibiotics and return trips to the operating theatre, increasing antibiotic resistance and reducing future available treatments. They are extremely costly and require long lengths of stay in hospital and lots of nursing time. In low and middle-income countries, they are at least twice as common as they are in high-income countries. Rates of antibiotic resistance are also higher, meaning they are even harder to treat. Wound infections affect both adults and children and since people of working age can't return quickly to work, their families and communities are also adversely affected.  FUTURE PROPOSED TRIAL: We aim to perform a large, multi-country randomised controlled trial looking at two simple interventions to reduce wound infections after emergency bowel surgery. Emergency surgery is the most common, demanding and burdensome type of bowel surgery in low and middle income countries (LMICs). Since the burden of wound infections is highest in these countries compared to other places in the world, this research proposal if timely and focussed toward a clinical need in LMICs. Wound infections are complex and testing two interventions at the same time may show even greater effect than just testing one, and it also improves value for money. The trial will be large and require around 2000 patients.  NEED FOR A DEVELOPMENT GRANT: Before we can submit a highly competitive, world-class grant proposal, we need to perform some development work to ensure that we are prepared. This work will develop a final, tangible plan for that application, and engage local doctors and research staff into the process. We aim to:  1. Hold on site meetings in participating countries, led by local surgeons, to engage a range of staff and shortlist interventions which could be tested in their units. 2. Deliver on-site research skills training that will increase local research capacity and leadership. 3. Perform small observational studies to ensure that sites have a strategy to follow-up patients during the main trial. 4. Hold our first face-to-face collaborator group meeting, to develop a finalised protocol based on the selected interventions, that can be used for a main trial application. This meeting will also allow us to foster the strongest possible relationships between our new collaborative and improve team working. NEED FOR MORE SURGICAL RESEARCH: There is a lack of surgical research across the world, partly driven by a lack of doctors regularly performing surgical research. This initial proposal will allow is to form a collaborative that could last for many decades. We predict that this network will enable us to deliver more trials into the future, of larger sizes and complexity. In the UK, we have developed hub and spoke research networks to deliver similar trials. We aim to transfer these research skills from the NHS to LMICs to help disseminate best surgical research practice widely. This will also speed recruitment to trials and maximise generalizability.       "description_narrative":["Kidney disease in Low and Middle income countries is under recognised and severely under resourced. The reasons include lack of clinical expertise, diagnostic capabilities, and even if recognised the means to treat chronic, remitting disease without specialist drugs and technology is rarely possible. The incidence of one of the commonest types of renal disease (in adults and children), idiopathic nephrotic syndrome (INS), appears considerably higher in LMICs than in the developed world. This is likely contributed to by a combination of infectious triggers and genetic background, though other factors remain unknown.  The biological understanding of glomerular disease has been revolutionised in recent years by study of the glomerular podocyte, the specialised cell type that is the primary target of damage in INS. The potential therefore is to use compelling biological advances to develop technologically sophisticated laboratory assays that can rapidly be tested on large local populations of patients in different LMIC settings. This will lead to directly applicable classification of disease according to new molecular and genetic findings, and hence targeted and more effective health care.  This proposal brings together different groups of researchers with different strengths, and will enable knowledge transfer between all partners. The host laboratory in the UK is a world leader in podocyte biology and INS population genetics, and has well-established links with key leaders in nephrology in several LMICs. In the UK we have established in nephrology a national patient registry (www.rarerenal.org) and biorepository, with widespread clinician and patient buy-in that is recognised as innovative This proposal aims to extend this concept internationally, to build a vibrant and cohesive network of academic centre leads in three LMICs with the UK centre as a hub. Each centre will be helped to build the infrastructure and expertise required to carry out biomarker and clinical trials studies on large and currently poorly understood cohorts of patients. The downstream benefits will be (i) establish a team of LMIC centres trained consistently in laboratory research/diagnostic techniques and clinical database/biobank establishment (ii) transfer of cutting edge translational biology advances from the UK laboratory (and elsewhere) to establish novel laboratory assays locally (iii) share clinical trials methodology that will maximise impact of laboratory findings from large cohorts of patients.       "description_narrative":["Aquatic animals such as fish and shellfish are an increasingly important source of protein for the growing world population. In 2014 the contribution of aquaculture to supply food for human consumption overtook that for wild-caught fish for the first time. Fish diseases are still considered to be a major constraint to aquaculture globally and it has been estimated that 10% of all farmed aquatic animals are lost because of infectious diseases alone, amounting to >10 billion USD in losses annually on a global scale. Farmed fish are often kept in intensive systems where antibiotics are heavily used. Effective vaccines exist for use in Atlantic salmon aquaculture and the use of antibiotics has been reduced by over 99%. In comparison, although many other species (e.g. carp and tilapia) are well established cultured species, there are few vaccines available, and antibiotics are still heavily used. Aquatic animals are increasingly traded worldwide, partly due to growing awareness for balanced diets and healthy eating in high-income countries and as a vital source of quality animal protein in low and middle income countries (LMIC). This creates opportunities and supports livelihoods in countries such as Bangladesh, Egypt and Vietnam, that together contribute to 9% of the global aquaculture production.  Despite the importance of antibiotic use in aquaculture and the size of the global trade, the contribution of aquaculture to the burden of consumer exposure to antibiotic resistance genes is currently not being addressed. Reasons why this issue is currently not been tackled include; i) the complexity of global food systems that involve different actors (producers, suppliers, intermediaries) spanning different countries across the world, ii) the lack of knowledge of the so called \"drivers\" of resistance in aquatic systems (i.e. the influences, e.g. through animal husbandry and farm management, that result in the emergence of resistance in bacteria) and, iii) the fragmented responsibilities in global food systems. However, it is important that antibiotic resistance in aquaculture is addressed as a global issue, because hazards emerging in producer countries will have public impact on consumers elsewhere through global trade and travel. Also, opportunities to learn from progress made in European fish farming remain underexploited. To address this complex challenge we need innovative research strategies that combine different disciplines and approach the system as a whole rather than its individual components in isolation. Our plan for the full proposal stage is to apply research methods that allow us to integrate societal, legal, governance and economic drivers in addition to biological risk, creating new approaches to better describe, analyse and assess the resilience of a global food system. In the first phase of the project, two workshops will be conducted to develop the model framework and plan the activities for the second phase. These workshops together with a review of published studies, reports and interviews with key stakeholders will allow us to map the different components of the system, the drivers, responsibilities and governance and to identify knowledge gaps for the main project. Our focus will be on selected fish species farmed in Bangladesh, Vietnam and Egypt, all of which are traded internationally. In the full-stage proposal, intervention strategies to reduce antibiotic use will be developed and implemented and their impact assessed using novel approaches.       "description_narrative":["In common with many Western countries, the Eastern Mediterranean Region (EMR) has a major problem with obesity and the associated public health issues. The World Health Organization (WHO) has been developing new health policies to lower intakes of sugar, saturated fats, trans fats and salt with the aim of reducing the risk of non-communicable diseases (diabetes, cardiovascular disease and obesity) in children and adults. To implement these policies successfully it is essential to have high quality food composition and food intake data to underpin research into the links between diet and health and to enable effective dietary monitoring and policy development. Most EMR countries use international food composition tables that include foods consumed in western diets with different nutrient composition, especially for mixed and traditional dishes. Some of these data from other countries are outdated and do not reflect the composition of foods currently consumed in EMR countries, meaning that calculated nutrient intakes are inaccurate. Methods of measuring dietary intake also need to be up-to-date and use appropriate food information. This project intends to provide technical and scientific support for researchers (mainly food and nutrition experts) in LMIC countries in the EMR, and will also provide training in the use of improved standardized methods for production and management of food composition and food intake data and also identify appropriate biological markers of food nutritional value and measurements of health outcomes. A review will be undertaken of food and nutrient intakes in the LMICs to develop strategies for reducing sugary and fatty foods in these diets based on new WHO guidelines. The Quadram Bioscience Institute, that aims to develop solutions to worldwide challenges in human health, food and infectious disease and non communicable disease, will lead the project in partnership with the WHO EMR office. The latter will help to coordinate and co-align regional activities to ensure public health priorities are targeted effectively. Three main activities will underpin capacity building in the EMR with a focus on 10 target LMICs (Morocco, Tunisia, Egypt, Jordan, Palestine, Sudan, Lebanon, Pakistan, Iran & Afghanistan). The project will build on existing links with the WHO EMR and will develop and extend activities with LMIC in the EMR. Four main activities are planned: (1) Training and knowledge exchange workshops; (2) Short-term exchanges of researchers; (3) Development and application of harmonised protocols for pilot sample collection, food analysis, data generation and creation of online regional food composition datasets; (4) Development of possible dietary and other strategies to support reduction in intake of sugary and fatty foods. The main outcomes will be to build capacity and practical skills in the LMIC to improve production, management and use of food composition and dietary intake data, and biomarkers. Knowledge of European and International standards will be disseminated, with support from WHO, to experts across the region. Networks of expertise within the region will also be an important outcome leading to increased collaboration and effective data sharing. The project will enable more efficient use of better quality, region specific, food composition and intake data that can be used for research, monitoring and policy development. Designing suitable foundations to sustain the loads typically applied by wind turbines represents a particular challenge. Most potential locations for wind turbines in Africa are in tropical zones where fluctuation in ground water level is severe. The cycling of water levels means that many deposits of interest are unsaturated for at least part of the year. Unsaturated soils exhibit complex mechanical behaviour, coupled to changes in water content.       "description_narrative":["Earthquakes, storms, floods, and conflict cause untold damage to infrastructure, services, agriculture and livelihoods. The destruction of housing is often most visible, and the most devastating to the families, with often hundreds of thousands of homes destroyed. Householders themselves are invariably the first to respond; they are never passive, and signs of shelter recovery are very apparent from the first days after a disaster. The majority - as many as 80 or 90% - receive little or no assistance from the international community and are the main drivers of their own recovery process. This process has been termed 'self-recovery'. Previous GCRF awards supported interdisciplinary teams led by ODI in collaboration with CARE International UK, BGS, UCL and Loughborough University. They developed the understanding of the self-recovery process and advanced the theoretical basis for humanitarian assistance to the process of supporting self-recovery in practice. In large part as a result of this work, self-recovery is now a strategic element of the humanitarian shelter sector discourse with many disaster responses purporting to support this inevitable process. In recognition of the value of a people-centred approach that moves away from the convention product-based modalities, the CARE Philippines post-cyclone self-recovery project won the 2018 World Habitat Award in acknowledgment of its self-recovery focus. Sixteen thousand houses were built, each one unique and reflecting the priorities and means of each family. https://www.world-habitat.org/world-habitat-awards/winners-and-finalists/post-haiyan-self-recovery-housing-programme/ The self-recovery approach that respects the primacy of agency and choice aligns well with current humanitarian and development concerns such as the 'humanitarian development nexus', the Grand Bargain's emphasis on localisation, participation and cash-based responses, the Sendai Framework on Disaster Risk Reduction, the Sustainable Development Goals and the Global Shelter Cluster strategy. This is further explained in the Business Case. However, very little has been done to develop guidance for practice. With the exception of the protocol 'Informing Choice for Better Shelter' that delivers a guide for the development of technical educational material, there are no published tools nor guidelines that provide an implementation framework for self-recovery in shelter projects. This project addresses this gap. CARE International has over 70 country offices across the world, all of them in ODA eligible  countries. All of the proposed activities will take place two hazard prone countries where there is a CARE presence and/or an activated coordination system in the form of a country level shelter cluster. The potential regions to be selected include South Pacific islands, East Africa, Central America, Philippines and SE Asia the precise locations to be dependent on ongoing and new disaster responses. In all instances the research team will be working with the Global Shelter Cluster, country level clusters and/or CARE International country teams. These will all be ODA eligible countries. The protocol developed will be adapted to become a preparedness tool for the specific ODA eligible country context.
