However, increased connectivity also carries the potential to introduce significant new societal challenges in regard to benefits and their (possibly unequal) distribution, as well as challenges for socio-cultural resilience and the mobilisation and protection of intangible and tangible local cultural heritages. Rapid changes pose threats to the conservation of traditional practices but also opportunities to develop them as resources (such as craft production and tourism sites) and also mobilising and developing them further as heritage assets. All too often, rapid and externally driven development has commodified local cultures, to be packaged for sale - with both control and most economic benefits going to outside developers. This is especially true of mountain, borderland communities whose interests are very often subordinated to state imperatives for territorial control and security. The proposed GCRF network will address such needs and help to preserve the local cultural heritages of formerly isolated mountain towns and, furthermore, keep these cultural heritages alive - as integral aspects of modern societies with clearly articulated pathways of cultural continuity. The network will aim specifically to develop capacities for interventions that focus on empowering (formerly) remote mountain towns and cities, enabling them to maintain greater control over their own cultures, development, and economic benefits. The network takes this forward in the context of huge infrastructural development in terms of planned transit corridors to ask how this hard infrastructural development intersects with built heritage and the soft infrastructure of intangible heritages and culture. The current level of infrastructural planning largely has been driven at national levels by strategic regional visions, and most often has ignored or inadequately incorporated the needs and wishes of local communities. The resilience of local cultures to the 'shock' of the largescale development and increased connectivity currently affecting and rapidly changing the target region needs enhancing. Current ways of life and socio-cultural practices may be profoundly disrupted by all these changes, even as they bring new opportunities. Current infrastructural developments are introducing many, new economic imperatives, new contacts and new pressures and new demands on fragile socio-ecosystems systems - including encounters with tourists and traders alike, with all the new possibilities to sell products more widely. The destruction of housing is often most visible, and the most devastating to the families, with often hundreds of thousands of homes destroyed. Householders themselves are invariably the first to respond; they are never passive, and signs of shelter recovery are very apparent from the first days after a disaster. The majority - as many as 80 or 90% - receive little or no assistance from the international community and are the main drivers of their own recovery process. This process has been termed 'self-recovery'. Previous GCRF awards supported interdisciplinary teams led by ODI in collaboration with CARE International UK, BGS, UCL and Loughborough University. They developed the understanding of the self-recovery process and advanced the theoretical basis for humanitarian assistance to the process of supporting self-recovery in practice. In large part as a result of this work, self-recovery is now a strategic element of the humanitarian shelter sector discourse with many disaster responses purporting to support this inevitable process. In recognition of the value of a people-centred approach that moves away from the convention product-based modalities, the CARE Philippines post-cyclone self-recovery project won the 2018 World Habitat Award in acknowledgment of its self-recovery focus. This is further explained in the Business Case. However, very little has been done to develop guidance for practice. With the exception of the protocol 'Informing Choice for Better Shelter' that delivers a guide for the development of technical educational material, there are no published tools nor guidelines that provide an implementation framework for self-recovery in shelter projects. This project addresses this gap. CARE International has over 70 country offices across the world, all of them in ODA eligible  countries. All of the proposed activities will take place two hazard prone countries where there is a CARE presence and/or an activated coordination system in the form of a country level shelter cluster. The potential regions to be selected include South Pacific islands, East Africa, Central America, Philippines and SE Asia the precise locations to be dependent on ongoing and new disaster responses. Kyrgyzstan in Central Asia is exposed to the hazards of earthquakes. The tectonic collision of the Earth's plates is creating huge mountains in this region. These mountains are created from earthquakes on large faults along the northern Tien Shan mountain range. Kyrgyzstan's' capital, Bishkek, lies on top of one of these major fault lines and is home to a million people. In the past, major cities that exist along the northern edge of this mountain range, such as Almaty to the east, have been destroyed in large earthquakes at the end of the 19th Century. This occurred when they were relatively small towns. The impact today from similar sized earthquakes would have a much more devastating effect if it were to strike the major city of Bishkek. The rapid expansion of cities in poorer countries has meant that a large number of buildings are not strong enough to be resilient to earthquakes. We want to help the Government ministries in Bishkek, such as the Ministry for Construction and also for Emergency Situations, to be able to better assess the potential impact for earthquakes to strike the city in future. We will do this by providing them with estimates of how many people may die in future earthquakes, how many buildings will be damaged or collapse, and how much such an earthquake will cost financially. We will also provide maps of where we think the city will be most affected by different types of earthquakes, as small nearby earthquakes can have as big an impact as distant large ones. This will enable the Kyrgyz government ministries to target where in the city key buildings, such as schools and hospitals, should be reinforced, as well as to better plan where new housing estates should be built, and also to enforce the seismic building codes to make sure the buildings are built better. In order to provide this latest information, we will be working with the Institute of Seismology in Bishkek, whose responsibility it is to provide these kinds of estimates of seismic risk. We are therefore working directly with the organisation that has the mandate to provide information to the government and by doing so we will ensure that our work will also have an impact. We are also going to train the institute staff to be able to update these estimates of losses so that they have the capacity to continue this work once we are no longer working on this project. It is important to be able to keep updating the estimates of losses and maps of seismic risk. Cities are constantly enlarging. If these cities lie in earthquake prone areas, such as the Kyrgyz capital of Bishkek, this growth increases their exposure to seismic hazards, increasing the risk that people face. Often some of the best views over a city come from higher ground, but this high ground is created by faults that build mountains. The city of Bishkek is expanding southwards as the urban population grows, and homes are now being built right on top of these fault lines. Being very near to a fault increases the amount of shaking if an earthquake happens, and therefore increases the chances of the building collapsing, injuring or killing the occupants. As well as the increased exposure to earthquake hazards, we are also discovering more active faults in the region through mapping out the fault lines and identifying past earthquake ruptures. It is important to incorporate this new information into the estimates of seismic hazard, as some very large earthquakes are known to have struck the region in the past. Therefore we will include this recent scientific information into our estimates of seismic hazard. We are working with other partners, such as the Global Earthquake Model Foundation, which was created to serve the public good through collaboration, openness and transparency by providing credible assessments of seismic hazard and risk. The tectonic collision of the Earth's plates is creating huge mountains in this region. These mountains are created from earthquakes on large faults along the northern Tien Shan mountain range. Kyrgyzstan's' capital, Bishkek, lies on top of one of these major fault lines and is home to a million people. In the past, major cities that exist along the northern edge of this mountain range, such as Almaty to the east, have been destroyed in large earthquakes at the end of the 19th Century. This occurred when they were relatively small towns. The impact today from similar sized earthquakes would have a much more devastating effect if it were to strike the major city of Bishkek. The rapid expansion of cities in poorer countries has meant that a large number of buildings are not strong enough to be resilient to earthquakes. We want to help the Government ministries in Bishkek, such as the Ministry for Construction and also for Emergency Situations, to be able to better assess the potential impact for earthquakes to strike the city in future. We will do this by providing them with estimates of how many people may die in future earthquakes, how many buildings will be damaged or collapse, and how much such an earthquake will cost financially. We will also provide maps of where we think the city will be most affected by different types of earthquakes, as small nearby earthquakes can have as big an impact as distant large ones. This will enable the Kyrgyz government ministries to target where in the city key buildings, such as schools and hospitals, should be reinforced, as well as to better plan where new housing estates should be built, and also to enforce the seismic building codes to make sure the buildings are built better. In order to provide this latest information, we will be working with the Institute of Seismology in Bishkek, whose responsibility it is to provide these kinds of estimates of seismic risk. We are therefore working directly with the organisation that has the mandate to provide information to the government and by doing so we will ensure that our work will also have an impact. We are also going to train the institute staff to be able to update these estimates of losses so that they have the capacity to continue this work once we are no longer working on this project. It is important to be able to keep updating the estimates of losses and maps of seismic risk. Cities are constantly enlarging. If these cities lie in earthquake prone areas, such as the Kyrgyz capital of Bishkek, this growth increases their exposure to seismic hazards, increasing the risk that people face. Often some of the best views over a city come from higher ground, but this high ground is created by faults that build mountains. The city of Bishkek is expanding southwards as the urban population grows, and homes are now being built right on top of these fault lines. Being very near to a fault increases the amount of shaking if an earthquake happens, and therefore increases the chances of the building collapsing, injuring or killing the occupants. As well as the increased exposure to earthquake hazards, we are also discovering more active faults in the region through mapping out the fault lines and identifying past earthquake ruptures. It is important to incorporate this new information into the estimates of seismic hazard, as some very large earthquakes are known to have struck the region in the past. Therefore we will include this recent scientific information into our estimates of seismic hazard. We are working with other partners, such as the Global Earthquake Model Foundation, which was created to serve the public good through collaboration, openness and transparency by providing credible assessments of seismic hazard and risk. "description_narrative":["Tuberculosis (TB) is a major cause of disease and death globally. In 2015, WHO estimated there were 9.6 million TB cases and 1.5 TB deaths. Nearly 500,000 of these cases were resistant to two or more of the main drugs used to treat TB. New drugs, and combinations of drugs, are being developed to treat tuberculosis, as are new vaccines that may protect against disease in adults. Quantifying the burden of TB is fundamental to understanding its global epidemiology and for making appropriate resource allocation decisions. Most estimates of new TB case numbers each year rely strongly on the number of cases reported by countries in that year to WHO. Unfortunately, one in three TB cases are thought to go either undetected or unreported, so the number of cases reported underestimates the number of new cases. While one can correct for this, it is hard to know exactly how much to adjust the reported numbers. Some countries have good systems for recording causes of deaths, which can be used to estimate the number of deaths caused by TB. Increasingly, large and expensive prevalence surveys are being used to estimate the number of people with active disease in a population. These estimates are less subject to bias, but measure a different quantity. Little work has explored the best way of combining these three data sources. A major goal of this work is to use mathematical transmission models for burden estimation and provide a unified framework for all data. These models yield the number of new cases, deaths, and also the prevalence of disease. They explicitly represent disease transmission and so introduce a dependence between the number of new cases in different years. These models involve parameters evidenced from previous epidemiological work, but must be calibrated to learn from data on TB reports, deaths and prevalence. Calibration means adjusting imperfectly known model parameters in order to match observed model outputs to the data. This process provides a model that may be used to make predictions about burden, but may also teach us something about the underlying processes. Many of the parameters concerning the epidemiology and disease course of TB are quite uncertain, and this uncertainty is rarely represented fully in models needing calibration, but will be done in this project using statistical techniques that also allow comparison of different models' performance. TB burden estimation and calibration of transmission models are almost always carried out on a country-by-country basis. Many parameters describing disease progression are likely to be similar in different countries, even if their exact values differ for unknown reasons. Hierarchical modelling techniques allow such parameters to be correlated between countries. This can improve precision, particularly for countries with little data, as estimates can be informed by data from neighbouring countries. I will explore these techniques for the transmission model, and also in statistical modelling aiming to account for the observed patterns of drug-resistance. The transmission model will ultimately be extended to include different types of drug resistance. As new treatments and vaccines emerge, those with responsibility for public health will want to understand the potential impact these new technologies can have in terms of gains in health, and changes in spending. Producing cost-effectiveness and budget impact evidence requires a model that includes transmission, in order to account for indirect benefits accrued by avoiding secondary cases. We will use our model to provide guidance to decision-makers seeking to maximise health gain with limited resources.
