      "description_narrative":["Operational Research (OR) has significant potential to contribute to meeting Sustainable Development Goals (SDGs) and disaster risk management (DRM) objectives of developing countries. However, most ODA countries lack the research capacity to develop and apply relevant OR methodologies. The CREST-OR project is a collaboration between three of the UK's leading universities in the field of OR - Kent, Lancaster, and Southampton - and prominent universities in four Southeast Asian (SEA) countries - Vietnam, Indonesia, Laos, and Cambodia. CREST-OR will establish a long-term UK-SEA partnership to build research capacity in OR methodologies, improve community resilience to natural disasters, and achieve national SDGs and DRM objectives. SEA partners will benefit from advancing their OR research capabilities, and the formation of new institutional links among project partners. UK partners will benefit from increasing knowledge in the application of OR to international development and disaster management challenges. The project will support longstanding initiatives in SEA countries through engagement of policy-makers and practitioners at national and local levels (e.g. emergency response agencies, local authorities, and development NGOs).  CREST-OR will build capacity in the use of OR for solving challenges in four focal areas: disaster management, food security, resilient transport infrastructure and smart cities. These areas were identified with the SEA partners based on existing collaborations within the UK researchers and, most importantly, on development priorities of their respective countries. In SEA countries, rapid economic growth and infrastructural development has not been supported by sufficiently robust planning. Social and environmental priorities have been regularly forfeited, while population growth has intensified development challenges. Climate change is now raising disaster risks and threatening to derail development achievements, with the greatest effect felt by the poorest and most vulnerable segments of society. OR offers a powerful set of tools to support more informed, evidence-based decision-making by government agencies and NGOs to address challenges related to disaster risk and its impact on food supplies, infrastructure and cities development. By building OR research capacity in these areas, CREST-OR will contribute to address several UN SDGs: SDG2 (Zero Hunger), SDG9 (Industry, Innovation and Infrastructure), SDG11 (Sustainable Cities and Communities), SDG13 (Climate Action) and Priority-2 (Strengthening Disaster Risk Governance) of the Sendai Framework.  The project will undertake the following capacity building activities: - Capacity assessments to identify educational and research gaps in the knowledge and use of OR methodologies, especially in relation to the SDGs, and understand the policy landscape in which CREST-OR will operate. - Three workshops, including: a Scoping Workshop to develop a prioritised list of relevant research projects, which will form the basis of CREST-OR research agenda; an Academic Workshop with specialised research training sessions for SEA researchers at different career stages; a Final Workshop to share developed research project proposals, receive end-user feedback and plan future research strategy. - Secondment of SEA Early Career Researcher in UK partner universities to develop research proposals, take part in research activites, and receive training in advanced and contextualised OR methods (e.g. simulation, optimisation). - Short visits of UK academics to SEA institutions to consolidate understanding of local context, run short training workshops, and support the development of OR curricula and research units. - Attendance of SEA researchers to the UK OR Society annual conference to facilitate their engagement in international debates and integrate them into a wider research community. - Development of a website for research networking and knowledge exchange.","The Global Challenges Research Fund (GCRF) supports cutting-edge research to address challenges faced by developing countries.       "title_narrative":["Blood Bricks: Examining the Climate Change-Modern Slavery Nexus in the Cambodian Construction Industry"],
      "description_narrative":["Climate change and slavery: the perfect storm?' - this was the prescient headline of The Guardian (2013) which called for more international conversation on the links between these urgent threats to environmental and human security. This study forwards this call by examining the inter-linkages between climate change, different axes of structural inequality (e.g. gender, age), and vulnerability to trafficking into modern slavery. The project asks who is most at the 'receiving end' of climate change, is most likely to enter into modern slavery, and who has fewer capabilities and resources than others to adapt to climate change in alternative ways?\n\nThe research is based in Cambodia, the world's second most climate vulnerable country in 2014. This status derives not only from the heightened climate risks its faces in the form of floods and droughts, but also the lack of capacity to adapt and respond. Eighty percent of the population lives in rural areas with limited knowledge, infrastructure and opportunities; and more than 70 percent rely on agriculture that is heavily sensitive to climate change (UNDP 2014). In 2016, Cambodia also recorded the third highest proportion of modern slaves per capita in the world. Under these compelling set of circumstances then, the project focuses on the Cambodian construction industry as a means to examine how climate change facilitates trafficking into modern slavery and ongoing livelihoods within it.\n\nUK and Cambodian scholars will undertake challenging research that aims to combine qualitative interviews with construction industry informants and victims of modern slavery working in brick-kilns and construction sites; agro-ecological profiling, a quantitative household survey, and interviews in brick-kiln sender villages; and analysis of longitudinal secondary data (Cambodia Socio-Economic Study 2014).       "description_narrative":["Combination drugs for treatment of malaria deployed globally since the start of the 21st century have provided a substantial public health benefit. Malaria deaths across Africa in particular have plummeted since a peak in the 1990s, when failure of the drugs chloroquine and sulphadoxine-pyrimethamine precipitated a rise in infant and child deaths. A key component in all the currently effective drug combinations is the plant-derived chemical called artemisinin. The parasite-killing ability of artemisinin has recently been threatened by the emergence of reduced susceptibility in the Mekong region: this is manifest as a slowing of artemisinin clearance of P. falciparum from the blood of treated malaria patients. Although under combination treatments these patients should eventually experience a full cure in most cases, in this region the combination partner drugs are also failing in some patients. Careful monitoring and vigilance of malaria drug resistance is therefore needed. A parasite gene has been identified in Cambodia and the surrounding region which contributes to the loss of effectiveness of the artemisinin drug - this gene, which encodes a kelch-domain protein and is called K13, has accumulated a variety of mutations. These are strongly associated with the loss of effectiveness of artemisinin against these parasites. However, this same phenomenon has not yet been observed in African malaria parasites - in the small number of cases where drug treatment does not work, K13 mutations are not implicated as the cause.     In the past 2 years, gene-editing studies by ourselves and others have successfully demonstrated a direct effect of 3 genes in reducing susceptibility to artemisinin : pfap2mu, pfubp1 and pfcoronin. Variants of these genes elicit increased parasite survival in vitro in the ring-stage artemisinin survival assay (RSA). In the case of trafficking adaptor potein subunit AP-2mu, we have shown that the role of this protein in P. falciparum is unlike in other organisms as it does not interact with clathrin, but is associated with an as yet unknown compartment in the cell that appears to also contain K13, although we found no evidence of direct interaction between AP-2mu  and the K13 protein. There is, however, good evidence of direct interaction of AP-2mu with a different kelch protein , K10. Polymorphisms in the gene encoding the K10 protein have been previously identified as a genetic component of parasites from the Mekong region in which the K13-dependent reduced artemisinin susceptibility originally arose. This work was carried out by PhD student Ryan Henrici, who completed his doctoral studies in 2018 and has moved on.   We now request support to extend this work, using gene editing to test the effect of new mutations. In preliminary studies of both field samples and UK isolates, we have already identified a number of such new mutations in our current five genes of interest - pfk13, pfap2mu, pfubp1, pfcoronin and pfk10 (see Case for Support). For each new isolate (either from the UK or from our collaborator Dr Bismarck Dinko in Ho, Ghana) that is adapted to culture, we will fully test drug susceptibility, and genetically characterise them using genome sequencing. In gene-editing experiments we will then directly measure the impact of each candidate gene variant in both established laboratory lines and in our new cultures from UK and Ghanaian patients; any variant proven to generate a drug susceptibility phenotype may therefore be contributing to changing patterns of drug susceptibility, and could be developed as a surveillance marker for use in malaria endemic areas. Our work will thus assist our understanding of resistance to our current combination drugs, and help us to devise strategies for deploying these drugs carefully in both the UK and Africa, to maximise their useful life in curing malaria patients.","The Global Challenges Research Fund (GCRF) supports cutting-edge research to address challenges faced by developing countries.       "description_narrative":["Antibiotic resistance has been termed the greatest menace of the current century by the World Health Organization. If appropriate measures to mitigate antibiotic resistance are not taken immediately, it is estimated that millions of people will die, in the near future due to complications arising out of inability to treat resistant infections and most of this burden will fall on the poor, underdeveloped part of the world.   The main drivers of antibiotic resistance are antibiotic use and antibiotic residues in the environment. Both generate resistant bacteria. Appropriate monitoring and mitigation of both is neglected in poorer countries due to inadequacy of awareness, infrastructure, economics, political will, socio-cultural dimensions and a myriad of other factors. Resistance knows no geographical or political boundaries; it spreads quickly all over the world. So, not only the poorer human beings from underdeveloped countries suffer, these countries also become 'hot spots' of resistance generation and spread.   Cambodia is the poorest country in the Greater Mekong Sub-Region and continues to recover from a prolonged civil war which saw its institutions and infrastructure destroyed, which resulted in the loss of most of its educated personnel. With support of international partners, Cambodia is rapidly developing;  Cambodia was one of the countries, which achieved the millennium development goals in reducing infant and child mortality. The proposed provinces for the antibiotic resistance study and intervention are among the most remote and underserved both within the formal health and agricultural systems and rely for care on personnel who have minimal training for both care of patients and animals. When a person or animal becomes ill, medicines including antibiotics, often of dubious quality are bought from local pharmacies or market stalls and administered by the persons themselves. Human antibiotics are often given to animals and vice versa. Due to the deficit of trained health personnel, Cambodia has developed a cadre of village level healthcare workers, who have minimal training and little supervision to treat villagers or their sick animals. Little care is taken in disposing of antibiotics, which must be resulting in huge amount of residues in Cambodian environment.   The proposed project will investigate and map the current status of antibiotic use, antibiotic resistance and antibiotic residues in the target area and would examine the drivers for these in these remote areas, involving various stakeholders including human and animal healthcare providers and policy makers. We would involve the newly instituted laboratory and personnel in the Royal University of Agriculture and Veterinary Medicine as well as our partners, the Ministry of Health and the Ministry of Fisheries and Agriculture.       "description_narrative":["What does it take to roll-out radical cure with primaquine regimens of vivax malaria in Cambodia, Laos, Vietnam, and Bangladesh? Without radical cure more than half of all patients with vivax malaria suffer one or often many repeated relapses. Not only does relapse cause suffering, loss of income and misery, people with relapsing malaria are also infectious and contribute to the continued transmission of vivax malaria. Vivax malaria relapse can be prevented by the administration of drug regimens which include primaquine. These drug regimens are underused because they can potentially trigger hemolysis (rupturing of red blood cells) in people with an inborn error in an enzyme (G6PD), which can be severe enough to require blood transfusions. Two important developments make it now easier to administer radical cure for vivax malaria. First, several companies have developed point-of-care sophisticated devices (biosensors) to measure G6PD enzyme activity. Using biosensors health care providers can detect patients with an enzyme deficiency and exclude them from treatment with primaquine. Second, a recent study has shown that a 7-day primaquine regimen is as safe and as good at clearing vivax malaria as the traditional 14-day drug regimen. We think that patients are more likely to complete a 7-day course than a 14-day course.   These two developments, biosensors and 7-day primaquine regimens, should lead to the widespread uptake of radical cure with primaquine. However, many health care providers and the policymakers (who make the guidelines health care providers follow) are not aware of these recent developments. Furthermore, the currently available evidence may not convince them. One way to convince them may be to start discussions on the preparation and eventual results from large demonstration trials. We are planning a large trial in which all vivax patients in selected villages receive a primaquine regimen. We will compare the incidence of vivax malaria in villages where primaquine radical cure is routinely used with the incidence of vivax malaria in villages where primaquine is not used, according to current national guidelines.  We expect that people with G6PD deficiency can be reliably detected at the point of care and excluded, that the primaquine regimens are well tolerated in G6PD-normal patients and that continuous treatment of vivax cases with radical cure will reduce and eventually stop the transmission of vivax malaria.   The demonstration trial is a major undertaking and critically depends on the choice of an appropriate G6PD test, the right primaquine regimen, the agreement of the proposed study community and the policy makers. To make sure the conditions are promising we are proposing here the funding of a development grant. We are planning to use the grant for a multidisciplinary preparatory study. The three main component of the proposed study are:   1) To interview relevant policymakers and stakeholders on what they think of the roll-out of the radical cure and what evidence would convince them to support the roll-out.  2) We will compare the performance of the available G6PD biosensors with the traditional standard tests in the diagnosis of the enzyme deficiency and select the best-performing biosensors that can be reliably and feasibly used by village health workers.  The village health workers already diagnose patients with vivax malaria using rapid diagnostic test. In addition, they will diagnose G6PD deficiency using the biosensors to make sure that biosensors are helpful in field conditions. 3. We will explore which is the acceptability by health workers and patient adherence to the primaquine regimen at the village level. 4) We will find out the most appropriate sites for the trial.       "description_narrative":["\nDengue is the fastest-growing mosquito-borne viral\ninfection in the world today. It is present in over\n150 countries, and approximately 40 percent of\nthe world’s population now live in countries where\ndengueis a daily risk.\nThe D-MOSS project is developing a dengue fever\nearly warning system that generates several months\nadvance warning of likely dengue outbreaks\nutilising Earth Observation (EO). An advance\nwarning on dengue risk, could help public health\nofficials plan cost-effective actions in advance and\nmove away from a reactive system where disease\ncontrol interventions occur after a dengue outbreak\noccurs.\nOne of the key D-MOSS components is a water\nassessment module that provides the additional\nbenefit of improving water management in\ntransboundary river basins. Seven of the nine major\nriver basins that drain to Vietnam are transboundary\nin nature and it is estimated that some two-thirds of\nVietnam’s water resources comes from neighbouring\ncountries, making water management challenging.\nIn recent years countries upstream of Vietnam have\nincreased their water use and Vietnam is currently\nfacing increasingly negative impacts from the water\npolicies of upstream countries. The development\nof an EO‑based water availability system will help\nthe Vietnamese Government to improve their\nwater resources monitoring and management in\ntransboundary river basins.\nThe dengue fever and water management\nchallenges are similar in other countries in South\nAsia and as of June 2019, the project has been\nextended to cover Cambodia, Laos, Malaysia, Sri\nLanka, Thailand and The Philippines.","This work will provide a tool that enables advance warning of likely dengue outbreaks, allowing public health authorities to mobilise resources to those most in need. The project will also provide forecasts of dengue fever under a range of climate change scenarios. The system will link Earth Observation data with climate forecasting and a land-surface model to predict for the first time the impacts of various elements (such as water availability, land-use, climate), on the likelihood of future dengue epidemics.
