      "description_narrative":["Women in developing countries worldwide face a high and unacceptable burden of adverse pregnancy outcomes. Curable, genital sexually transmitted infections (STIs) are major contributors to this disease burden in many resource-limited settings, but the majority of these infections go untreated.  Health services are unable to diagnose them because of limited laboratory facilities and the cost of conventional laboratory-based STI diagnostic tests. In the absence of such resources, diagnosis has relied on the clinical strategy known as 'syndromic management' that is recognised as having very poor sensitivity, particularly for women, in whom the vast majority of these STIs are asymptomatic. Newly available, highly accurate and easy to use point-of-care tests for the first time offer a real solution to this long-term health problem in developing countries.   We will undertake a world-first field trial to investigate the effectiveness of a comprehensive point-of-care testing and treatment strategy for curable STIs to improve pregnancy outcomes in resource-limited, high-burden settings. Our trial will also be the first to investigate new molecular point-of-care tests, based on the GeneXpert technology, for the routine diagnosis of chlamydia, gonorrhoea and trichomonas infections in resource-limited settings. This newly available technology will allow trained health staff to make correct diagnoses and to provide curative treatment during routine antenatal clinic visits.   Papua New Guinea is a unique environment in which to conduct this research because of the conjunction of (a) a high burden of both adverse pregnancy outcomes and curable STIs among pregnant women; and (b) recent and extensive experience by our team in the conduct of health intervention studies involving pregnant women and their children.   We will conduct a randomised trial among 4200 pregnant women attending 12 antenatal clinics in Eastern Highlands, Madang and Milne Bay Provinces. Participating clinics and their catchment communities will be randomly allocated to receive either routine antenatal care (control arm) or point-of-care STI testing and immediate treatment (intervention arm). Women in both arms of the trial will undergo an obstetric ultrasound examination to accurately estimate the duration of their pregnancy; be asked to attend a minimum of four antenatal follow-up visits; and receive a postnatal follow-up visit by the research team within 48 hours of giving birth. We will compare birth outcomes between antenatal women who were offered point-of-care STI testing and treatment with those who were not.   We will measure the difference in pregnancy outcomes among women in each arm of the trial in terms of the proportion of preterm birth (birth less than 37 weeks gestation) and low birth weight (birth weight less than 2500 grams). We will also evaluate the cost-effectiveness, health system implementation requirements, and acceptability of antenatal point-of-care STI testing and treatment compared to routine antenatal care in order to maximise the impact of our research and to inform future roll-out.       "description_narrative":["More than a quarter of the global human population depends on small-scale subsistence agriculture for their livelihoods. As these farmers rely on crops from a small cultivated area, they are highly vulnerable to extreme weather events. One such event is El Niño, a periodic reversal of ocean currents in the Pacific Ocean with pronounced global impacts on weather. Papua New Guinea (PNG) is a lower middle income country in the western Pacific, which is particularly hard hit by El Niño events. Since 85% of the population depend on subsistence farming in family \"food gardens\", El Niño can cause widespread damage to crops and livelihoods, directly through drought and frost; and indirectly through crop pests and diseases. Farmers can respond partially, by changing the crops they grow and where they grow them, or by collecting alternative foods from surrounding forest. When extreme events like El Niño occur they set off a series of related effects across ecological and social systems. For example, crops subjected to drought may be more susceptible to pests and thus suffer greater damage, leading to lower yields and reduced food. In turn, people may use forest products to supplement their food, which could degrade the forest, reducing its ability to buffer the village against future drought.  Our project will develop an integrated approach to investigating linkages between ecological and socioeconomic impacts of the 2015 El Niño on subsistence farming in PNG. We will focus on food gardens from 200m to 2700m elevation on Mount Wilhelm. We will compare ecological and social changes caused by the El Niño in villages at different elevations, and whether people with different abilities to cope with droughts (e.g. those with larger/smaller gardens) were affected differently. We will collect data on how the current El Niño is affecting crops through drought and frost, and by altering the number of insect pests and the severity of crop diseases. We will assess how El Niño might cause increased reliance of farmers on food collected from surrounding forests, how this affects forest biodiversity, and the ability of the forest to support resilient food gardens by providing pest control.   We will gather villagers' perceptions of the impacts of El Niño on their livelihoods and wellbeing, and on how they coped. For example, we will ask whether there are things they would like to have done to cope, but were unable to do (e.g. changing to drought-resistant crop varieties), and discover the barriers to employing these coping strategies. Together with local villagers and our PNG collaborators, we will develop options to help support villagers and surrounding ecosystems during future El Niño events. We will ask villagers which option they prefer, and how they would change their behaviour under different future scenarios. For example, if El Niño events became more frequent, would they change their crops permanently? If the forest was reduced due to logging, would this change their response to El Niño? Using our results we will build a model of how the forest, food gardens and people's livelihoods interact, and use it to talk to villagers and the government about the likely impact of different interventions to support villagers. We will then propose ways to make their livelihoods more sustainable and resilient, for future El Niño events.  Our project builds on a long-term collaboration with the New Guinea Binatang Research Center, in which local research assistants are trained to collect ecological data. The project extends our research to include social dimensions. We will train the assistants to collect information about people as well as insects, to build a long-term project that can support villages to become more resilient to future climate change and build scientific capacity within PNG.       "description_narrative":["Earthquakes, storms, floods, and conflict cause untold damage to infrastructure, services, agriculture and livelihoods. The destruction of housing is often most visible, and the most devastating to the families, with often hundreds of thousands of homes destroyed. Householders themselves are invariably the first to respond; they are never passive, and signs of shelter recovery are very apparent from the first days after a disaster. The majority - as many as 80 or 90% - receive little or no assistance from the international community and are the main drivers of their own recovery process. This process has been termed 'self-recovery'. Previous GCRF awards supported interdisciplinary teams led by ODI in collaboration with CARE International UK, BGS, UCL and Loughborough University. They developed the understanding of the self-recovery process and advanced the theoretical basis for humanitarian assistance to the process of supporting self-recovery in practice. In large part as a result of this work, self-recovery is now a strategic element of the humanitarian shelter sector discourse with many disaster responses purporting to support this inevitable process. In recognition of the value of a people-centred approach that moves away from the convention product-based modalities, the CARE Philippines post-cyclone self-recovery project won the 2018 World Habitat Award in acknowledgment of its self-recovery focus. Sixteen thousand houses were built, each one unique and reflecting the priorities and means of each family. https://www.world-habitat.org/world-habitat-awards/winners-and-finalists/post-haiyan-self-recovery-housing-programme/ The self-recovery approach that respects the primacy of agency and choice aligns well with current humanitarian and development concerns such as the 'humanitarian development nexus', the Grand Bargain's emphasis on localisation, participation and cash-based responses, the Sendai Framework on Disaster Risk Reduction, the Sustainable Development Goals and the Global Shelter Cluster strategy. This is further explained in the Business Case. However, very little has been done to develop guidance for practice. With the exception of the protocol 'Informing Choice for Better Shelter' that delivers a guide for the development of technical educational material, there are no published tools nor guidelines that provide an implementation framework for self-recovery in shelter projects. This project addresses this gap. CARE International has over 70 country offices across the world, all of them in ODA eligible  countries. All of the proposed activities will take place two hazard prone countries where there is a CARE presence and/or an activated coordination system in the form of a country level shelter cluster. The potential regions to be selected include South Pacific islands, East Africa, Central America, Philippines and SE Asia the precise locations to be dependent on ongoing and new disaster responses. In all instances the research team will be working with the Global Shelter Cluster, country level clusters and/or CARE International country teams. These will all be ODA eligible countries. The protocol developed will be adapted to become a preparedness tool for the specific ODA eligible country context.
