      "description_narrative":["\nDengue is the fastest-growing mosquito-borne viral\ninfection in the world today. It is present in over\n150 countries, and approximately 40 percent of\nthe world’s population now live in countries where\ndengueis a daily risk.\nThe D-MOSS project is developing a dengue fever\nearly warning system that generates several months\nadvance warning of likely dengue outbreaks\nutilising Earth Observation (EO). An advance\nwarning on dengue risk, could help public health\nofficials plan cost-effective actions in advance and\nmove away from a reactive system where disease\ncontrol interventions occur after a dengue outbreak\noccurs.\nOne of the key D-MOSS components is a water\nassessment module that provides the additional\nbenefit of improving water management in\ntransboundary river basins. Seven of the nine major\nriver basins that drain to Vietnam are transboundary\nin nature and it is estimated that some two-thirds of\nVietnam’s water resources comes from neighbouring\ncountries, making water management challenging.\nIn recent years countries upstream of Vietnam have\nincreased their water use and Vietnam is currently\nfacing increasingly negative impacts from the water\npolicies of upstream countries. The development\nof an EO‑based water availability system will help\nthe Vietnamese Government to improve their\nwater resources monitoring and management in\ntransboundary river basins.\nThe dengue fever and water management\nchallenges are similar in other countries in South\nAsia and as of June 2019, the project has been\nextended to cover Cambodia, Laos, Malaysia, Sri\nLanka, Thailand and The Philippines.","This work will provide a tool that enables advance warning of likely dengue outbreaks, allowing public health authorities to mobilise resources to those most in need. The project will also provide forecasts of dengue fever under a range of climate change scenarios. The system will link Earth Observation data with climate forecasting and a land-surface model to predict for the first time the impacts of various elements (such as water availability, land-use, climate), on the likelihood of future dengue epidemics. The dengue forecasting tool will also include a water assessment module, delivering the additional benefit of improving water management in Viet Nam's transboundary river basins."],

      "description_narrative":["The Philippines is a nation of approximately 104\nmillion people for whom locally caught fish provide\n56% of their total animal protein intake annually.\nIncreasing the supply of affordable, edible fish is\nvital for improved nutrition and food security. Yet\nsome estimates indicate that illegal, unreported\nand unregulated fishing (IUUF) is depriving the\nPhilippines of fish stocks valued at $1bn-$2bn\nannually.\nProject Verumar tackles this problem head-on. It\nsupports the Philippine Government in conserving\nand sustainably using their oceans, seas and\nmarine resources. This is achieved by integrating\nand working in concert with the Bureau of\nFisheries and Aquatic Resources (BFAR) and other\nstakeholders to scope and introduce new satelliteenabled\ncapabilities. These enhance fisheries\nmanagement and enforcement activities, and\ndirectly enable illegal fishing to be countered with\nimproved efficiency.\nIn addition, the project undertakes capacity\nbuilding to impart the latest remote sensing skills\nand techniques to ensure that the impact of project\nVerumar is enduring, while also embedding the\nPhilippine government’s forward-thinking gender\nequality principles and policies across the project,\nensuring women’s full and effective participation\nand visibility.\nSupporting work areas are developing options to\nsustain the impacts which are being realised under\nthe IPP, exploring and initiating further international\npartnership opportunities and ensuring effective\ncommunications such that knowledge developed\nduring the Project Verumar IPP is highlighted to key\naudiences in the wider space and development\nsectors.","The project will develop a satellite-enabled, machine-learning solution that will support the Philippine partner Bureau of Fisheries Aquaculture Resources (BFAR) with the identification of illegal and unregulated fishing and management of fisheries.  This will reduce overfishing in Philippine waters and make the national fishing grounds more sustainable and productive in the long term.  \n\n \n\nBy delivering an improved and fully integrated monitoring, control and surveillance system, the project will enable the targeting of specific interventions to provide immediate value at the strategic and operational level to BFAR.       "description_narrative":["With current global changes in diet and lifestyle, even low- and middle-income countries are experiencing the 'double burden' of undernutrition and overweight. Both conditions can be found within communities, families and even individuals at different times over their life course. It is well established that malnutrition in pregnancy, resulting in an infant born with low birth weight, can increase the risk of diseases such as diabetes, heart disease and cancer in adulthood. However, the consequences of malnutrition after birth are much less studied. Severe acute malnutrition in childhood, indicated by extreme thinness, remains common in Africa and Asia. In addition, substantial numbers of adult patients with tuberculosis or HIV, diseases which are common in Africa and Asia, may become severely malnourished. We are interested in diabetes, which in Africa and Asia affects people at younger age and lower weight than in Europe. There is evidence from epidemiological studies that severe malnutrition in childhood and possibly in adulthood increases the risk of later diabetes but there is little information as to the mechanisms involved. We wish to focus on the pancreas which is a key organ in digestion and metabolic processes, especially in relation to diabetes. We will investigate pancreas size, microscopic structure, hormone and digestive enzyme production, and the body's response to these hormones among groups of people in Tanzania, Zambia, India and the Philippines. These groups have participated in the research team's previous studies of malnutrition and were severely malnourished before birth, as children, or as adults. They now live in places with a wide range of access to foods high and fat and sugar which could affect their risk of diabetes. We will use modern clinical methods to compare their pancreas function to that of never-malnourished controls at each site. We will use advanced statistical methods to understand the links between early malnutrition and later diabetes, taking into account the factors often associated with diabetes such as current overweight and high intakes of fat or sugar.  The research will lead to improved understanding of both the long-term consequences of malnutrition and the presentation and underlying metabolism of diabetes in Africa and Asia. Thus, it will lead to improved health care for both malnourished and diabetic people even if we find no important link between early malnutrition and later diabetes.","The Global Challenges Research Fund (GCRF) supports cutting-edge research to address challenges faced by developing countries.       "description_narrative":["Tuberculosis (TB) is a major cause of disease and death globally. In 2015, WHO estimated there were 9.6 million TB cases and 1.5 TB deaths. Nearly 500,000 of these cases were resistant to two or more of the main drugs used to treat TB. New drugs, and combinations of drugs, are being developed to treat tuberculosis, as are new vaccines that may protect against disease in adults.  Quantifying the burden of TB is fundamental to understanding its global epidemiology and for making appropriate resource allocation decisions. Most estimates of new TB case numbers each year rely strongly on the number of cases reported by countries in that year to WHO. Unfortunately, one in three TB cases are thought to go either undetected or unreported, so the number of cases reported underestimates the number of new cases. While one can correct for this, it is hard to know exactly how much to adjust the reported numbers. Some countries have good systems for recording causes of deaths, which can be used to estimate the number of deaths caused by TB. Increasingly, large and expensive prevalence surveys are being used to estimate the number of people with active disease in a population. These estimates are less subject to bias, but measure a different quantity. Little work has explored the best way of combining these three data sources.  A major goal of this work is to use mathematical transmission models for burden estimation and provide a unified framework for all data. These models yield the number of new cases, deaths, and also the prevalence of disease. They explicitly represent disease transmission and so introduce a dependence between the number of new cases in different years. These models involve parameters evidenced from previous epidemiological work, but must be calibrated to learn from data on TB reports, deaths and prevalence. Calibration means adjusting imperfectly known model parameters in order to match observed model outputs to the data. This process provides a model that may be used to make predictions about burden, but may also teach us something about the underlying processes. Many of the parameters concerning the epidemiology and disease course of TB are quite uncertain, and this uncertainty is rarely represented fully in models needing calibration, but will be done in this project using statistical techniques that also allow comparison of different models' performance.   TB burden estimation and calibration of transmission models are almost always carried out on a country-by-country basis. Many parameters describing disease progression are likely to be similar in different countries, even if their exact values differ for unknown reasons. Hierarchical modelling techniques allow such parameters to be correlated between countries. This can improve precision, particularly for countries with little data, as estimates can be informed by data from neighbouring countries. I will explore these techniques for the transmission model, and also in statistical modelling aiming to account for the observed patterns of drug-resistance. The transmission model will ultimately be extended to include different types of drug resistance.  As new treatments and vaccines emerge, those with responsibility for public health will want to understand the potential impact these new technologies can have in terms of gains in health, and changes in spending. Producing cost-effectiveness and budget impact evidence requires a model that includes transmission, in order to account for indirect benefits accrued by avoiding secondary cases. We will use our model to provide guidance to decision-makers seeking to maximise health gain with limited resources.
