      "description_narrative":["The notion that the international community has a duty or \"responsibility to protect\" is not new.   It has been raised not only in the context of armed conflict but also when addressing economic, social and cultural rights. In both contexts, the concept includes: the duty to respect; the duty to protect and; the duty to fulfil, that is, to work actively to establish political, economic, and social systems as well as infrastructure that provide access to the guaranteed right to all members of the population.  While the responsibility to fulfil these obligations fall primarily to states within their own borders where a state fails or lacks capacity, that responsibility increasingly falls to the international community.   Member states have, in turn, attempted to respond to the needs of individuals living in Fragile and Conflict-Affected States (FCAS) by developing protection interventions. Yet what is clear from existing academic research and UN reports, determining the most effective and appropriate protection interventions that affirm rights and mitigate physical or psychological harm poses a number of significant challenges for the international community.\n\nIn focusing on conflict-related sexual violence (CRSV), this proposal will address one such challenge and fill what we argue to be significant gaps in current research on male CRSV survivors.  Drawing on the work of Jill Stauffer (2015) and Philipp Schultz (2018), we will apply Stauffer's concept of 'ethical loneliness,' defined as the \"isolation one feels when one, as a violated person or as one member of a persecuted group, has been abandoned by humanity, or by those who have power\" (1) to male CRSV survivors.  We argue that in focusing on this subject group and adopting this conceptual framework, our research will engage four of the designated thematic areas of this call-Impact of Violations, Impact of Absent or Ineffective Protection Programming, Impact of Recognition Protection, Impact of Targeting on groups excluded from targeted protection/response.  In his Ugandan study of sexual violence, Schultz argues that providing a better understanding of the \"effects of externally imposed and gender-specific silencing\" has a \"wider utility beyond male sexual violence\" allowing us to better understand and address the multiple needs of \"survivors of political and wartime gendered violence more broadly.\"   In focusing on male CRSV, the research and methods proposed will address each prong of the 'egg model' and:\n\n1. Provide a comprehensive base for understanding the factors that lead to male CRSV, and its patterns of abuse;\n2. Examine the impact of the initial violation and subsequent harm from the invisibility of male CRSV including: lack of access to appropriate, culturally and gender sensitive treatment and support for survivors and their family; impact on\n        societal cohesion of their community and; any further violence that may manifest.\n3. Develop key strategies to address the layers of invisibility of male CRSV, and facilitate access to critical support and recovery services, including sexual and reproductive health (SRH), other medical care, Mental Health and Psycho-Social\n        Support (MHPSS), protection, and access to justice/reparations.\n\nIn each of these tasks, the research questions are designed to interrogate the drivers of invisibility (stigma, taboos, risks, gendered norms, absence or exclusion from policies and programming) which can leave male SV survivors behind. This, in turn, effects cohesion, stability and recovery within the wider community (including families - specifically women and girls, and community recovery post-conflict), and longer term, perhaps inter-generational transmission which has been seen for other atrocity crimes.       "description_narrative":["Tuberculosis (TB) is a major cause of disease and death globally. In 2015, WHO estimated there were 9.6 million TB cases and 1.5 TB deaths. Nearly 500,000 of these cases were resistant to two or more of the main drugs used to treat TB. New drugs, and combinations of drugs, are being developed to treat tuberculosis, as are new vaccines that may protect against disease in adults.  Quantifying the burden of TB is fundamental to understanding its global epidemiology and for making appropriate resource allocation decisions. Most estimates of new TB case numbers each year rely strongly on the number of cases reported by countries in that year to WHO. Unfortunately, one in three TB cases are thought to go either undetected or unreported, so the number of cases reported underestimates the number of new cases. While one can correct for this, it is hard to know exactly how much to adjust the reported numbers. Some countries have good systems for recording causes of deaths, which can be used to estimate the number of deaths caused by TB. Increasingly, large and expensive prevalence surveys are being used to estimate the number of people with active disease in a population. These estimates are less subject to bias, but measure a different quantity. Little work has explored the best way of combining these three data sources.  A major goal of this work is to use mathematical transmission models for burden estimation and provide a unified framework for all data. These models yield the number of new cases, deaths, and also the prevalence of disease. They explicitly represent disease transmission and so introduce a dependence between the number of new cases in different years. These models involve parameters evidenced from previous epidemiological work, but must be calibrated to learn from data on TB reports, deaths and prevalence. Calibration means adjusting imperfectly known model parameters in order to match observed model outputs to the data. This process provides a model that may be used to make predictions about burden, but may also teach us something about the underlying processes. Many of the parameters concerning the epidemiology and disease course of TB are quite uncertain, and this uncertainty is rarely represented fully in models needing calibration, but will be done in this project using statistical techniques that also allow comparison of different models' performance.   TB burden estimation and calibration of transmission models are almost always carried out on a country-by-country basis. Many parameters describing disease progression are likely to be similar in different countries, even if their exact values differ for unknown reasons. Hierarchical modelling techniques allow such parameters to be correlated between countries. This can improve precision, particularly for countries with little data, as estimates can be informed by data from neighbouring countries. I will explore these techniques for the transmission model, and also in statistical modelling aiming to account for the observed patterns of drug-resistance. The transmission model will ultimately be extended to include different types of drug resistance.  As new treatments and vaccines emerge, those with responsibility for public health will want to understand the potential impact these new technologies can have in terms of gains in health, and changes in spending. Producing cost-effectiveness and budget impact evidence requires a model that includes transmission, in order to account for indirect benefits accrued by avoiding secondary cases. We will use our model to provide guidance to decision-makers seeking to maximise health gain with limited resources. We will also analyse sources of uncertainty in the model to identify future research that would have most value in increasing the precision of burden estimates and in reducing decision uncertainty around the introduction of new interventions.","The Global Challenges Research Fund (GCRF) supports cutting-edge research to address challenges faced by developing countries.       "description_narrative":["WHO and the G20 have identified the growing threats of Anti-Microbial resistance (AMR) as a major concern that will define the future of global health. Despite these urgent calls, the emergence of AMR in settings of war and distress migration has not been systematically explored. Case reports from Syria, Iraq, Libya, Yemen, and Afghanistan have shown the proliferation of AMR in combatants and civilians injured in these protracted conflicts. With regional conflicts spreading across state borders as well as one of the largest global refugee crises in decades, AMR in the context of conflict has come to pose a serious threat both regionally and internationally. So began penicillin in the Second World War: antibiotics arose in war. Today, in the context of long-running military conflicts we see harbingers of the end of antibiotics. The core question underpinning this proposal is how war, particularly weapons and the industrialised, urbanised context of contemporary conflicts, drives antibiotic resistance by contaminating the environment and the human and non-human organisms that live there. So far, there has been no systematic or holistic consideration of the environmental health impacts of contemporary conflicts conducted in cities.  Our program draws together scholars working in the fields of medicine, anthropology, history of science, ethics, epidemiology, microbiology, molecular biology, and environmental sciences to examine the specific intersection of antibiotic resistance and war. Rather than focus on antibiotic resistance as a universal problem afflicting modern societies in general, we focus first on the impact of global conflict on antibiotic resistance more holistically, and second on the case of multi-drug resistant Acinetobacer baumanii (MDRAB), initially reported by American military surgeons under the moniker Iraqibacter, and that has been identified recently by the WHO as a \"critical pathogen\" for research and the development of new antibiotics. We will focus on a number of specific countries - Iraq, Syria, Palestine, Yemen, and Lebanon-places with history of protracted conflicts and with different, yet overlapping, ecologies of war.  The potential global health significance of conflict-related heavy metal mediated antimicrobial resistance is enormous and warrants further study.       "description_narrative":["Additional GCRF funding to the Higher Education Funding Council for Wales to support Welsh higher education institutes (HEIs) to carry out ODA-eligible activities in line with their institutional strategies. ODA research grants do not represent the full economic cost of research and therefore additional funding is provided to Welsh HEIs in line with their research council grant income. In FY19/20 funding was allocated to Aberystwyth University, Bangor University, Cardiff University and Swansea University. In FY19/20, the funding was used to fund: the full economic cost of existing ODA eligible activities (e.g. already funded by GCRF); small ODA-eligible projects; fellowships to ODA-eligible researchers; and to increase collaboration and impact. 53 ODA-eligible countries have been reported as benefiting from the funded work, with Brazil and India the most frequently mentioned. By region, the largest number of projects were based in the LDC’s (Least Developed Countries) in Asia, South America, and East Africa, with only a few projects in the middle-income countries such as Kazakhstan, Kyrgyzstan and Georgia.","This activity contributes to the aims and objectives of BEIS ODA Research and Innovation funds the Newton Fund and the Global Challenges Research Fund.
